BRafeNHS APPLICATION SCHEDULES FOR ACADEMIC YEAR 2022-2023


Applications for Grade 7 and Grade 11 Technical-Vocational-Livelihood (TVL) Track, Information Communication and Technology (ICT) Strand are extended until May 31, 2022.

Application period may be extended depending on the availability of slots.

ADMISSIONS POLICY

The De La Salle Santiago Zobel School (DLSZ) is a Catholic school. Non-Catholics are admitted provided they participate in all religious activities that are required of the students. No exemption shall be granted by reason of membership in or affiliation with a different religion. 

A student’s application will be given due course when the student-applicant meets all the academic requirements and has none of the disqualification prescribed by the School, the Department of Education 2010 Revised Manual of Regulations for Private Schools in Basic Education and the DLSZ Student Handbook as amended and such other rules promulgated by the School.

The school reserves the right at any time to refuse, to admit, or to re-admit students under certain conditions as enrolment in DLSZ is a privilege.

QUALIFICATIONS

  • A graduate from a public elementary school;
  • From financially challenged families within Muntinlupa and its adjacent cities;
  • With annual family income below PhP 350,000.00; and
  • With a General Average of 85% or higher, no grade below 82% in all subjects and a Conduct Grade of at least B (80-84)

REQUIREMENTS

  • Clear Scanned Copy of Birth Certificate issued by the Philippine Statistics Authority (PSA);
  • Clear Scanned Copy of Report Card (DepEd School Form 9) with the student’s LRN from the previous academic year and the current academic year with at least the 1st quarter grades;
  • One (1) recent 2×2 photo (close up, white background);
  • Recommendation Form (generated through the online application form; to be accomplished by the Adviser/School Counselor and emailed to Ms Jenifer Bumanglag, Assistant to the Registrar for BRafeNHS, at bumanglagje@dlszobel.edu.ph)
  • Signed (a) Application Confirmation Slip, (b) Data Privacy Statement Consent Form and (c) Waiver for Non-Catholics, if applicable (generated through the online Application Form);
  • Proof of payment for the Testing Fee of Php100.00
  • Scanned Copy of the parent’s/guardian’s proof of annual income. (Please choose ONE ONLY.)

A. If parents/guardian are employed:

  • latest Payslip (2 months);
  • ITR (Income Tax Return) or Form 2316; and
  • Contract of Employment and latest remittance slip (for OFWs).

B. If parents/guardian are unemployed:

  • Certificate of Tax Exemption (issued by the BIR); or
  • Social Case Study Report (issued by the DSWD)

NOTE: Incomplete requirements will not be processed.

APPLICATION PROCEDURE

Fill out and submit the online application form via the Admissions Portal (https://forms.dlszobel.edu.ph/admissions/). Download the Recommendation Form, Application Confirmation Slip, Data Privacy Statement Consent Form and Waiver for Non-Catholics (for non-Catholics only) generated through the online form.

Pay the non-refundable Application Fee of PhP 100.00 through the different online payment facilities as indicated below.

1. All payments to the school may be made online via internet banking by using your bank’s mobile app or website to transfer funds to the school’s Union Bank account with the following details:

 2. DLSZ online payment facility (https://pay.dlszobel.edu.ph) using your credit card or online banking accounts with the following details:

 3. Over the counter payments can be made at any Union Bank branch using the Bills Payment Slip or Deposit Slip with details below:

An official receipt shall be issued after confirmation of payment. The official receipt may be claimed from the school’s Finance Office after we resume normal school operations.

Submit all application requirements through the Admissions Portal.

Wait for test permit to be emailed and click this DLSZ-BRafeNHS Online Admissions Test Step by Step Guide, then please be available on the day and time of online assessment.

5 (Final Interview)

Applicants, together with their parents/guardians, who successfully passed the entrance test will be subjected for final interview. The Admissions Personnel will call for these applicants regarding the schedule and venue of the final interview.

6 (Release of Test Results)
  • Check the status of application via the Results Portal (https://forms.dlszobel.edu.ph/application-result/) on May 13, 2022.
  • Inquiries made through calls or email regarding the test results will NOT be entertained.
  • Parents of accepted applicants should confirm enrollment on the dates indicated in the Acceptance Letter.

PROCEDURE FOR CONFIRMATION OF ENROLLMENT

Submit all enrollment requirements through the Admissions Portal.

(a) original copy of PSA Birth Certificate

(b) original copy of student’s Final Report Card

 

The original copy of these documents must be submitted to the Registrar’s Office via courier service using the address below:

Pay the Enrollment Fee of Php 520.00 through the payment facilities mentioned above.

Send a copy of the proof of payment for the enrollment fee via email to enroll@dlszobel.edu.ph and nhsenrollment @dlszobel.edu.ph.

IMPORTANT NOTE:

  • Parents Orientation will be announced after enrollment.
  • The applicant is considered officially enrolled ONLY when he/she has submitted the following: 
  1. Official Receipt of Enrollment Fee
  2. Final Report Card (DepEd School Form 9)
  3. Form 137 (DepEd School Form 10)

IMPORTANT NOTE:

All submitted documents in compliance with the application and enrollment requirements shall become the property of DLSZ and are not to be returned to the applicant.
