UPCAT application forms will be available at the Seniors’ School Counseling Office (SCO) starting May 29, 2013. Interested applicants must bring four (4) pcs. recent and identical 2″ x 2″ photos.
NO photos, NO immediate processing of UPCAT Form 2 (Secondary School Record).
