We encourage parents/guardians of non-school bus riders to fill out the Transport Registration Form (TRF) as soon as possible.
The TRF covers application for DLSZ Car Stickers, Fetcher’s IDs, and may serve as DLSZ’s endorsement to AAVA’s Carpool Passes/Car Stickers.
Procedure:
- Download the (updated v2) PDF Form here. The form may be filled out manually or electronically using Adobe PDF Reader.
- Submit one set (two copies) of the TRF per carpool group to the Campus Development and Services Office (CDSO) located at the Upper Ground Floor of the St. Mutien Marie Building (above the DLSZ Bookstore).
- Attach the following requirements:
- One photocopy of OR/CR for each vehicle
- Two 1×1 photos of each student
- Two 1×1 photos of each fetcher
Note:
- Application for the AAVA Carpool Passes/Car Stickers is subject to AAVA’s guidelines.
- Attendance to the DLSZ Traffic Management Seminar is a prerequisite to the following:
- Issuance of DLSZ Car Stickers and Fetcher’s IDs
- Endorsement for AAVA Carpool Passes/Car Stickers
- The DLSZ Traffic Management Seminar will be held on May 29, 2015 (Friday), 2:00 pm at the Debbie Decena Auditorium. Additional schedules will be announced through the school website.
