The DLSZ Parents’ Bulletin No. 22 reports on the outcome of the July 9, 2015 meeting between De La Salle Santiago Zobel School (DLSZ) and the Ayala Alabang Village Association (AAVA). \u00a0Attended by\u00a0parent representatives and school administrators (on the side of DLSZ), and the community leaders, the residents of the immediate vicinity of DLSZ and the residents of Ayala Alabang Village (on the side of AAVA), the meeting tackled the issuance of AAVA stickers, the Memorandum of Agreement between DLSZ and AAVA,\u00a0and related issues. \u00a0View DLSZ Parents’ Bulletin No. 22<\/a>
\nAll parents\/guardians applying for AAVA stickers (carpool, special, outliers) should submit the following:<\/p>\n

    \n
  1. OR\/CR and other pertinent documents (only if they have not given these to the school last June when we released the Transport Registration Form (TRF)<\/li>\n
  2. AAVA Sticker Application Form – ONLY ONE form per carpool group<\/span>; one form also for each special and outlier sticker applicant. \u00a0AAVA will handle the notarization of the forms and will charge a fee of approximately P100 per form<\/span>.<\/li>\n
  3. Sticker Application Information Sheet – ONLY ONE sheet per carpool group<\/span>; one sheet also for each special and outlier sticker applicant.<\/li>\n<\/ol>\n

    The school will receive through the Gate 2 guards the duly accomplished AAVA sticker application forms even on Saturday, July 11 and Sunday, July 12. Deadline for submission of forms is on Monday, July 13, 2015. All forms will then be reviewed and endorsed by DLSZ before these are forwarded to the AAVA office.<\/span>
