The DLSZ Safety and Security Office releases information regarding sticker application for AY 2019-2020:<\/p>\n

1. DLSZ sticker is the same for both Alabang and Vermosa campuses.<\/span><\/p>\n

2. New Stickers will be available starting <\/span>April 10, 2019.<\/span><\/p>\n

3. Sticker payments will be accepted only <\/span>after due endorsement<\/span> from the Safety and Security Office (SSO).<\/span><\/p>\n

4. No endorsement shall be given until after attending the <\/span>Traffic Management Seminar<\/span> (TMS) for :<\/span><\/p>\n

A. Parents with Traffic Violation Report (TVR)<\/p>\n

B. Parents of new students<\/span><\/p>\n

5. The TMS schedule is from 9:00 to 11:00 am of <\/span>May 29<\/span> and <\/span>31, 2019<\/span> only.<\/span><\/p>\n

6. A maximum of two (2) stickers<\/span> can be availed per family.<\/span><\/p>\n

7.\u00a0 Application forms with <\/span>incomplete attachments<\/span> will NOT be endorsed and processed. <\/span><\/p>\n

A. Photocopy of valid OR\/CR<\/span><\/p>\n

B. Photocopy of valid government ID<\/span><\/p>\n

C. Other docs in lieu of CR, as applicable:<\/span><\/p>\n

a. Photocopy of Sales Invoice (for newly purchased car)<\/span><\/p>\n

b. Photocopy of Notarized Deed of Sale <\/span><\/p>\n

c. Photocopy of Notarized SPA (for relative of the car owner)<\/span><\/p>\n

d. Photocopy of Deed of Assignment or its equivalent (for company cars).<\/span><\/p>\n

8. Rate of stickers<\/span><\/p>\n

A. First Car <\/span> 300.00<\/span><\/p>\n

B. Second Car <\/span> 400.00<\/span><\/p>\n

EASY STEPS:<\/b><\/p>\n

1. Fill out the DLSZ Sticker application form.<\/span><\/p>\n

2. Attach the required documents.<\/span><\/p>\n

3. Submit them to the SSO for evaluation and endorsement.<\/span><\/p>\n

4. Pay the sticker fee at the Finance Office.<\/span><\/p>\n

5. Present the receipt to the SSO for encoding. <\/span><\/p>\n

6. Bring the car near the SSO (side of St. La Salle Building) for the sticker to be attached by the security personnel.<\/span><\/p>\n

NOTE:<\/b>
\nApplication for <\/span>AAVA endorsement<\/span> shall follow similar procedure, except for the ff:<\/span><\/p>\n

A. Payment to be done in AAVA office.<\/span><\/p>\n

B. Stickers are released by AAVA authorized personnel.<\/span><\/p>\n
