Please fill out the form manually.<\/li>\n<li>Submit one set (two copies) of the TRF per carpool group to the Campus Services Office (CSO) located at the Ground Floor of the St. Mutien Marie Building (old Accounting Office).<\/li>\n<li>Attach the following requirements:\n<ul>\n<li>One photocopy of OR\/CR for each vehicle<\/li>\n<li>Two 1\u00d71 photos of each student<\/li>\n<li>Two 1\u00d71 photos of each fetcher<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>Note:<\/p>\n<ul>\n<li>Application for the AAVA Carpool Passes\/Car Stickers is subject to AAVA\u2019s guidelines.<\/li>\n<li>Attendance to the DLSZ Traffic Management Seminar is a prerequisite to the following:\n<ul>\n<li>Issuance of DLSZ Car Stickers and Fetcher\u2019s IDs<\/li>\n<li>Endorsement for AAVA Carpool Passes\/Car Stickers<\/li>\n<\/ul>\n<\/li>\n<li>The DLSZ Traffic Management Seminar will be held on May 19 and 20, 9:00 am at the Center for Performing Arts (CPA) Lobby.<\/li>\n<\/ul>\n<p>&nbsp;\n<\/p><\/div>\n","protected":false},"excerpt":{"rendered":"<p>We encourage parents\/guardians of non-school bus riders to fill out the Transport Registration Form (TRF) as soon as possible.
